It’s a question you’ll get every time you see a new psychiatrist: What medications have you tried already?

If you’re one of the estimated 40 million people in the U.S. who take medications for your mental health, it can be difficult to keep track of what medications you have already tried in the past. You and your doctor will likely have to try a variety of different medications at different doses until you find one that works. Those medications may change over the years and 10 years later, the details get hazy.

To help you better keep track of your mental health meds (and to have a great document to give to your doctor or psychiatrist), we’ve created a two-page check list of many of the most common mental health meds currently prescribed. Tick off the ones you have tried in the past, and add dosage, dates, side effects and other important details next to the drug name or in the notes section.

You can save the image below or click on the link to download a PDF that you can fill out electronically and print. We want to make living with a mental health condition a little more manageable. This list can make answering at least one question about your health simpler.
